Brightlives offers Independent Clinical assessments for children and adults with complex care needs who receive funding support from the National Disability Insurance Scheme (NDIS). These are specifically designed for Participants who require a detailed assessment and documentation of these care needs, which then informs the NDIS of the hours of carer support required and the most suitable level of carers to deliver the care.

Our Clinical Consultants can provide advice and support for our clients and Disability Support Providers. Our Team are very passionate about supporting people with complex health needs and ensuring that they have thorough assessments and reports that reflect their needs. Having worked within the sector for many years, we understand the challenges of having complex health needs and can provide advice about best practices and understanding NDISC regulations.

CLINICAL ASSESSMENTS
COMPREHENSIVE ASSESSMENT
A comprehensive nurse assessment is a valuable tool for understanding a participants current and future health support needs. The nurse assessment incorporates all aspects of a person’s health including physical, psychological and cultural needs. The assessment reflects on assessments and recommendations from other health professionals such as Occupational Therapy, Physiotherapy and medical reports, creating an all-encompassing summary and providing a basis for planning and provision of care.
CONTINENCE ASSESSMENT
Our continence nurse consultant will come to your home, or meet with you in an online consultation, to discuss your continence needs. We can assist with continence assessment for NDIS funding applications and NDIS plan reviews, provide advice about continence management for adults and children and support you to choose the most appropriate continence products by arranging for samples of continence pads to be sent to you to try in the privacy and comfort of your own home.

Brightlives offers Independent Clinical assessments for children and adults with complex care needs who receive funding support from the National Disability Insurance Scheme (NDIS).

These are specifically designed for Participants who require a detailed assessment and documentation of these care needs, which then informs the NDIS of the hours of carer support required and the most suitable level of carers to deliver the care, which may or may not include nurses as part of the care team.
Participants who have had these Independent Nursing Assessments may be living at home with family, living independently with carer support or in a Group Home setting.

The initial meeting is usually an extensive interview with either a participant's family member or a representative or carer who knows the participant's needs very well. Then a head-to-toe style assessment with additional care domains is also explored.
The assessment also reflects on assessments and recommendations from other health professionals, such as Occupational Therapy, Physiotherapy and medical reports, and behaviour support, to create an all-encompassing summary and provide a basis for planning and provision of care. Recommendations are then made as part of the report about the number of hours and the level of carers that are suitable to provide support for the participant.
