An NDIS Continence Assessment is a specialised evaluation designed to support individuals with disabilities who may experience challenges with bladder and bowel control. Conducted by a qualified Continence Nurse, like Monica at Bright Lives, this assessment identifies a person’s unique continence needs and provides tailored strategies and recommendations to improve their quality of life.

What To Expect

NDIS CLIENTS:

  1. Referral Process:

    • You will be required to complete our referral form, which can be filled out by yourself or your support coordinator.

  2. Appointment Scheduling:

    • The team at BrightLives will reach out to you to schedule an appointment via Telehealth (Zoom).

    • A service agreement will be sent to you for signing.

  3. Pre-Appointment Preparation:

    • You will receive an email prior to your appointment containing a list of documents that will assist with your assessment.

    • It is recommended to send these documents before your appointment, if possible.

  4. Assessment Completion:

    • During the assessment, your nurse will gather the necessary information and conduct the evaluation.

  5. Report and Recommendations:

    • After completion of the assessment, your nurse will compile a report along with clinical recommendations.

    • This report will be emailed to you and your support coordinator for review.

  6. Optional Follow-Up Appointment:

    • An optional appointment will be offered to discuss the report and finalise any necessary changes or adjustments.

2. Improved Daily Living:  6 to 8 hours Nursing @ 163.91
