An NDIS Continence Assessment is a specialised evaluation designed to support individuals with disabilities who may experience challenges with bladder and bowel control. Conducted by a qualified Continence Nurse, like Monica at Bright Lives, this assessment identifies a person’s unique continence needs and provides tailored strategies and recommendations to improve their quality of life.

What To Expect
NDIS CLIENTS:

Referral Process:

You will be required to complete our referral form, which can be filled out by yourself or your support coordinator.

Appointment Scheduling:

The team at BrightLives will reach out to you to schedule an appointment via Telehealth (Zoom).

A service agreement will be sent to you for signing.

Pre-Appointment Preparation:

You will receive an email prior to your appointment containing a list of documents that will assist with your assessment.

It is recommended to send these documents before your appointment, if possible.

Assessment Completion:

During the assessment, your nurse will gather the necessary information and conduct the evaluation.

Report and Recommendations:

After completion of the assessment, your nurse will compile a report along with clinical recommendations.

This report will be emailed to you and your support coordinator for review.

Optional Follow-Up Appointment:

An optional appointment will be offered to discuss the report and finalise any necessary changes or adjustments.
